Jerry Ray Carlton II, age 42 of Bon Aqua, died May 21, 2015 at Centennial Medical Center in Nashville.Jerry was born February 15, 1973 in Hollywood, FL and was the son of Jerry Ray Carlton, Sr and the late Lena Weaver Carlton.He is survived by his wife; Carla Herman Carlton of Bon Aqua; four children: Jessica Lynn Carlton, Jerry Ray Carlton III, James Lee Carlton and Jacob Ray Carlton; his father: Jerry Ray Carlton Sr; two sisters: Linda Reed and Patty Maynard; and a brother: Brian Hawkins.His family will celebrate his life at a later date in Woodbury, TN.NASHVILLE FUNERAL AND CREMATION 615-256-1605
